Scholarship Application and Submission Form
The Chippewa Valley Chiropractic Alliance is pleased to announce a scholarship opportunity for students from or affiliated with the Chippewa Valley in North West Wisconsin. All students from the Chippewa Valley or affiliated with a Chiropractor in the Chippewa Valley are eligible.
This Scholarship is to be awarded annually and will be awarded in the sum of $500.00. The deadline for the scholarship application is December 1st, 2009 and will be awarded on December 31st, 2009. Award funds will be paid directly to the recipient's Chiropractic College to be credited to the student's account.

We do ask that the winner be willing to have their essay printed along with their picture on the Chippewa Valley Chiropractic Alliance's web page, as well as other publications.
